Create a form
Start a new form from a template, a blank form or just an endpoint, and learn how form keys and endpoints work.
Each form has its own endpoint, inbox, settings and integrations. Create one form per form on your site, so submissions stay separate and each form can have its own notifications.
Create a form
Choose Create a form on the dashboard or the Forms page. The form is created in the team you are working in, shown next to New form.
Give it a name and pick a starting point:

Templates come with fields, a title, a button label and a success message. You can change all of them in the form builder. Browse the templates on the templates page.

Forms without fields
A form doesn't need fields. Endpoint only and Blank form accept whatever your form sends and save every field as it arrives. Add fields later if you want validation, nicer labels in the inbox or a styled embed. See Fields and validation.
Form limit
The Free plan allows 3 forms per team. Pro has no limit. When a team reaches the limit, the New form dialog offers to upgrade or to manage your existing forms. Deleting a form frees up its slot.
Endpoint and form key
Every form gets a random key, and its endpoint is https://api.nisuform.com/s/ followed by the key:
https://api.nisuform.com/s/k3yz8q2m4n6p0r5t7v9wFind it on the Setup tab under Your endpoint, or in Settings, Endpoint.
The key is public by design. It appears in your page's HTML, and that is safe. It only lets people send submissions to that one form. It can't read submissions or change anything. To keep unwanted traffic out, use spam protection and access rules.
Rotate the key
If a form is getting abused, go to Settings, Endpoint and choose Rotate key. The form gets a new endpoint and the old one stops accepting submissions right away, so update your site with the new endpoint immediately. Submissions you already received are kept.
Name, icon and type
In Settings, General:
- Name is shown in your dashboard and in notification emails.
- The icon button next to the name picks an emoji for the form, shown in your forms list.
- Type is the kind of form: Contact, Waitlist, Feedback, RSVP or Custom. It is shown on your form cards and used in the AI tool prompt. Changing it doesn't change your fields.
Find a form
The Forms page lists every form in the team with its submission count. Search by name or key, and sort by newest, oldest, name or number of submissions. Submission counts leave out spam and test submissions.
